As a per diem Preschool Teacher Aide - Substitute, you will work with teachers in the management of a preschool classroom by providing quality care to ages 3-5 following Agency philosophy, program performance standards and curriculum guidelines. You will report to the Preschool Supervising Teacher and you will work onsite.
Responsibilities:
- Use daily routines and observations to meet the strengths and needs of children
- Work as a team member to facilitate the programs curriculum, procedures and philosophy.
- Assist in the implementation of weekly program plans.

POSITION OVERVIEW
As a Family Support Worker, you will work as part of the Short-term Services Program which was developed as an intensive case-management service to rapidly resolve the concrete and referral needs of families entering the Child Welfare System. By successfully resolving these issues within 90 days of admission, families are at a reduced risk for being referred to Long-term CPS. Services are provided in the office, home, school, and community. You will report to the Coordinator of Short-term Services and will work onsite with community-based travel required.
Responsibilities:

As a Home Based Crisis Intervention (HBCI) Specialist, you will support families and youth in crisis by providing in-home services. As a Part-Time Case Manager, Regional Permanency Resource Center you will provide supportive services and case management services to adoptive, guardianship and kinship families so they are able to make informed decisions with regards to services and supports that best meet their needs. Clinical Program Director

As the Clinical Program Director of Community Transition Time Program (CTTP), you will oversee a new model of care with a multi-disciplinary team that provides services and support to adolescent youth and their families utilizing residential, community and home-based services. The goal of this program is to keep adolescents out of unnecessary hospital stays and instead provide wraparound care and case management for themselves and their families where they need it, in their home or in a short-term residential setting when needed.
You will manage the program and staff, develop relationships with referral sources including local hospitals and county mental health providers, client referral systems, client treatment and case management, client financial viability, as well as insights into the marketing of program and updating the program systems. You will report to the Senior Director of Residential Services and will work onsite.
Responsibilities:
- Develop and manage relationships with local hospitals and mental health providers, care management providers, schools, primary care providers, housing authorities, and law enforcement agencies in the coordination of youth referrals into the program.
- Ensure coordination of child/youth discharge to the community, warm hand-off to community care management and facilitation of the use of the TRS as required for brief periods of stabilization.
- Oversee a short-term eight bed residential unit that provides 24/7 care.
- Be the spokesperson for this new program model and participate in collaborative meetings with the state and other service agencies, as well as educate the community about CTTP, outcomes and its theoretical background.
- Collaborate with the Local Governmental Unit (LGU), Director of Community Service (DCS)/Mental Health Commissioner and work with NYS OMH and other key agencies to provide training and support around the team's development.
- Consult and coordinate with hospital and program team members to ensure continuity and quality of care.
- Ensure the continuous quality and improvement of services and development of the program, including regular monitoring and reporting of program outcomes for agency and state mandates.
- Be the Lead on recruiting, hiring and onboarding team members.
- Provide overall supervision to the team and create rotating on-call schedule for after hours and weekend coverage by team members.
- Participate in on call rotation with other program directors.
- Monitor caseload size and coordinate program intakes and discharges.
- Provide training and supervision for the staff and participate in all required trainings as directed.
- Participate in OMH-directed training and technical assistance, including regular calls, learning collaboratives, etc.
- Ensure program staff are adhering to the fidelity of the program.
Requirements and Education:
- A Master’s degree and active NYS license in a helping profession (such as LMSW, LCSW, Licensed Marriage and Family Therapist, Licensed Psychologist, Licensed Mental Health Counselor, Licensed Creative Arts Therapist, etc.)
- At least 5 years of relevant clinical experience post Master's degree.
- At least 3 years of leadership or management experience required.
- A valid NYS Driver’s license and the ability to travel within program service area.
